Family Care Teams are interprofessional groups of health care providers who provide longitudinal, comprehensive, person-centered care to individuals in a region. Together with Quality of Care NL and the Office of Professional and Educational Development, CCHPE supports the interprofessional practice needs of Family Care Teams as a consultant.

The Family Care Teams Resource Hub

The Resource Hub is a website that curates interprofessional practice supports that can help Family Care Teams develop their collaboration skills and team performance. Visitors to the Hub can find

Family Care Team Foundations Program

The Foundations Program is a series of online, self-paced modules that anyone working in any Family Care Team can access to up their collaborative practice skills and knowledge. The Program also includes regular workshops that can be delivered online (for anyone across the province) or in person (with individual Family Care Teams). The module and workshop topics are outlined below, and although we have a recommended pathway through the modules (organized using Tuckman’s Stages of Group Development Model), any Family Care Team members can complete any of the modules to meet their specific and immediate needs.

Family Care Teams Webinar Series

About once per month, we host a 1-hour webinar on a topic of interest for Family Care Team members that introduces and discusses a primary care issue with an interprofessional collaborative practice lens. Our staff are joined by an expert in the topic. Webinars are accredited with the College of Family Physicians of Canada. You can find recordings of previous webinars on the Resource Hub website.
