Supervisory Reports

The supervisory committee will meet with the graduate student on an annual basis to review the progress of the student and recommend (or not) the continuation of the student in the graduate program. It is normally the responsibility of the supervisor to organize this meeting but it may also be arranged on the recommendation of the student. The purpose of the review is to assure that the student is completing course work at the appropriate graduate level and engaged appropriately in research (collecting and/or analyzing data or writing). The supervisory committee will also make recommendation for a switch from MSc to the PhD program. At the end of the supervisory committee meeting the supervisory report will be completed and signed by the student, supervisor and members of the supervisory committee in attendance. Any concerns will be noted on the supervisory committee report. This report will be submitted to the Associate Dean of - Graduate Studies (Medicine) for approval and forwarded to the Dean of the School of Graduate Studies. Any changes in the program of study, supervisory committee or thesis title will require submission of a Change of Program form (see below).

Please note: Supervisory Report forms maybe found online on the School of Graduate Studies website.
