Forms
There are several forms that MUN's medical learners are required to submit no later than August 14th, 2023. Learners will be able to upload these forms as part of their Brightspace Orientation Course once they have registered for their Phase 1 courses.
While the Office of Learner Well-Being and Success maintains records of completed forms and certificates, learners are encouraged to make copies for their own records.
Learners will not be able to submit their forms prior to the beginning of Orientation, however, they are still strongly encouraged to complete or acquire the following forms and security checks as soon as possible:
- Confidentiality Oath
- Eastern Health Oath Confidentiality
- Release of Information
- College of Physicians and Surgeons of Newfoundland and Labrador Payment Form
- Police Record Check and Vulnerable Sector Check
- Information for Section 2 of the Criminal Records Screen Certificate

Should a learner be unable to have their forms fully authorized or notarized appropriately, please COMPLETE THE FORMS TO THE BEST OF YOUR ABILITY AND SUBMIT THEM ON THE ORIENTATION PORTAL. Learners with incomplete forms should ensure that notify the Office of Learner Well-Being and Success.
Allowances and extensions will be granted to accommodate any COVID-19 related delays wherever possible. These allowances and extensions will ONLY be granted if the request is submitted to the Office of Learner Well-Being and Success.
PHIA Oath for Learners
As a requirement of the NL PUBLIC HEALTH INFORMATION ACT or PHIA (pronounced Fee-ah), all medical learners are also required to complete the Personal Health Information Act online training module before the end of Orientation. This online course explains important elements of the NL Personal Health Information Act and is targeted to those who have or will have direct or indirect contact with personal health information.
Leaners must register on the PHIA Online Education Course website and complete the"Direct Contact with Personal Health Information" course. Certificates of completion can then be uploaded as part of the Brightspace Orientation Course.
The PHIA oath/affirmation can be witnessed and signed at the Office of Learner Well-Being and Success once learners arrive on campus.
