Biosafety Permit Application
A MUN biosafety permit is required for all activities (research and teaching) that involve the use or manipulation of biohazardous agents, and materials containing such agents which are:

Conducted on University premises, or in a building or location administered by or under the control of the University,

Undertaken by University personnel (staff or students), or
-
Funded through University-administered funding.
A valid biosafety permit is required before the University will release/continue funds for any grant that involves biohazards.
IMPORTANT: Unit administrative heads must be made aware of the scope of work described in biosafety permit applications.
Biosafety permit application forms are now located on the EHS Assistant software (through my.mun.ca), within the "Permit applications/forms" section.
New Applicants/Renewals
New applicants: Completed application form, including the Biohazard Procedures Risk Assessment and Matrix for Assessment of Risk Group (the matrix is required only if the RG is unknown or other documentation is unavailable) must be forwarded to the BSO AT LEAST SIXTY (60) DAYS PRIOR TO THE DATE APPROVAL IS REQUESTED.
Renewals: Complete renewal applications (including procedures risk assessment and RG documentation) must be received by the BSO NO LATER THAN two (2) weeks PRIOR TO IBC meeting for which review is saught (see IBC meeting schedule ).
Current Biosafety Permit Holders - Amendments
For current biosafety permit holders who intend to modify or change aspects of their research with biohazards (eg. changes to projects/procedures, biohazards, locations, etc.), an amendment to their biosafety permit must be approved by the IBC. Applications for changes in biohazards and/or procedures will be reviewed at the next scheduled IBC meeting. Applications for changes in locations will be reviewed at the discretion of the BSO (inspection may be required).
